If you are a commercial driver in the Charlotte area, passing your DOT physical is essential to staying on the road and keeping your CDL active. Two of the most important parts of the exam are hearing and vision requirements. At Carolina MedWorks in Charlotte, NC, I perform thorough, compliant DOT exams to make sure you meet federal standards — and if there are concerns, I guide you on what steps to take next.
The hearing and vision standards are set by the Federal Motor Carrier Safety Administration (FMCSA). These requirements are in place to ensure drivers can safely operate commercial vehicles and respond to traffic conditions, warning signals, and road hazards.
DOT Vision Requirements
DOT physicals are designed to ensure that commercial drivers are medically qualified to operate commercial motor vehicles safely. Because many medications affect alertness, cardiovascular health, glucose control, and neurological function, the FMCSA sets specific standards for how medications are handled during evaluation.
Vision Testing in My Charlotte Office
During your DOT physical I will perform 3 vision tests.
1) Standard visual acuity testing using a Snellen eye chart
- Passing: 20/40 or better in each eye separately and in both eyes combined
Measured with or without corrective lenses (glasses or contacts)
2) Horizontal Field of Vision
- Passing: You must be able to see at least 70 degrees to the side (left or right) in each eye.
3) Color Recognition
- Passing: Must be able to recognize and distinguish traffic signal colors:
- Red
- Green
- Amber/Yellow
What If You Don’t Pass?
If you do not meet the vision standards during your exam, I may refer you to an optometrist or ophthalmologist for further evaluation. In some cases, drivers may qualify for a federal vision exemption through the FMCSA, but this requires additional documentation and review outside my office.
DOT Hearing Requirements
Commercial drivers must be able to perceive a forced whisper at 5 feet in at least one ear (with or without hearing aids). Alternatively, drivers can pass with an audiometric test showing:
An average hearing loss of no more than 40 decibels at 500 Hz, 1000 Hz, and 2000 Hz in the better ear.
Hearing Testing in My Office
At Carolina MedWorks in Charlotte, I only perform one hearing test.
1) The standard forced whisper test
Passing: You must be able to hear and repeat several letters and numbers spoken in a whisper from 5 feet away.
If you use hearing aids and meet the standard with them, you can still qualify. Your DOT medical certificate will indicate that hearing aids must be worn while driving.
If Additional Testing Is Needed
If hearing ability is borderline or unclear during the office screening, I may refer you to an audiologist for formal audiometric testing. Once documentation confirms you meet FMCSA standards, we can proceed appropriately.

Commercial driving demands constant awareness. You must be able to:
See traffic lights, brake lights, and signage clearly
Detect pedestrians and vehicles in your peripheral vision
Hear horns, sirens, and mechanical issues
Failing to meet these standards can delay certification, but many drivers qualify with corrective lenses or hearing aids.
