The Physical Exam Portion: What to Expect

The physical exam is a comprehensive assessment that evaluates various aspects of a driver’s health and takes approximately 40 minutes. Here’s a detailed breakdown of what the physical exam includes:

Medical History Review

Discussion of Past and Current Health Conditions: The exam starts with a review of your medical history. You’ll be asked about any past surgeries, ongoing medical conditions (like diabetes or hypertension), medications you are taking, and any history of substance abuse.

Review of Previous Injuries and Surgeries: Understanding any past injuries or surgeries is crucial to assessing potential long-term impacts on your driving abilities.

Family Medical History: Conditions that may run in the family, such as heart disease or diabetes, are also considered.

Vision & Hearing Test

Acuity and Peripheral Vision: To pass the DOT physical, you must have at least 20/40 vision in each eye with or without correction (glasses or contact lenses). The exam also checks peripheral vision to ensure you can see objects or people from the side without turning your head.

Color Vision: You’ll be tested to distinguish the colors red, green, and amber, as this is crucial for reading traffic signals and road signs.

Whisper Test: The DOT exam requires that you can hear a forced whisper from a distance of at least 5 feet, with or without a hearing aid. Alternatively, you can pass the hearing requirement by demonstrating the ability to perceive a minimum of 40 dB loss in your better ear.

Urinalysis

Testing for Blood Sugar and Protein Levels: A urine test screens for early signs of diabetes and other underlying health conditions. It is not a drug test but an essential part of the health screening process.

Blood Pressure and Pulse Rate

Hypertension Screening: Blood pressure and pulse are checked to screen for high blood pressure and irregular heartbeats. Your blood pressure must be below 140/90 in order to pass your exam.

Physical Examination

General Appearance: The examiner will look for any signs of a medical condition that might affect your ability to drive safely, such as severe obesity, jaundice, or respiratory issues.

Lungs and Chest: Listening to your lungs for any abnormal sounds or breathing difficulties.

Heart: Checking for irregularities in heart rhythm and other cardiovascular issues.

Abdomen: Examining for abnormalities that could indicate hernias or other medical conditions.

Musculoskeletal System: Evaluating posture, spinal alignment, and joint function to ensure you have the physical capacity to operate a commercial vehicle.

Mobility and Flexibility Tests: The exam includes checking the functionality of your limbs, which is crucial for operating vehicle controls. Any impairment in limb function can be a safety concern.
