DOT Blood Pressure Categories Explained

During your DOT physical in Charlotte, I measure your blood pressure and place it into one of three DOT categories:

Normal or Controlled Blood Pressure (Below 140/90)
If your blood pressure is under 140/90, you may qualify for a 2-year DOT medical certificate, assuming everything else on your exam is normal.

Stage 1 Hypertension (140–159 / 90–99)
Drivers in this range can usually receive a 1-year certification. This is very common and does not prevent you from driving.

Stage 2 Hypertension (160–179 / 100–109)
At this level, I can issue a one-time 3-month certificate. This gives you time to work with your primary care provider to lower your blood pressure. When you return with improved readings, you may qualify for a longer certification.

Stage 3 Hypertension (180/110 or higher)
Blood pressure at or above this level is considered disqualifying until it is brought down. Once controlled, you can be re-examined and potentially certified.

What Causes High Readings at DOT Exams?

I often see elevated readings due to stress, caffeine, nicotine, poor sleep, or “white coat” anxiety. That’s why I take the time to repeat measurements when appropriate and ensure they’re done correctly. However, the final reading must still meet DOT standards.

  • Avoiding caffeine and nicotine for several hours

  • Taking prescribed blood pressure medications as directed

  • Getting adequate sleep the night before

  • Bringing documentation if your blood pressure is being treated
